Arrangement Conference Checklist - The arrangement conference checklist is a list of information we will be covering during the arrangement conference. Please review this document to ensure you bring the necessary items and documentation with you.

Arrangement Worksheet- This arrangement worksheet is a fillable .pdf document that you can fill out, save, email and/or print. (May not be fillable/compatible with some mobile devices) This worksheet contains vital statistic information necessary for us to complete death certificates and certain permits. It also contains detailed information of how you would like for services to be conducted.

The mobile compatible form below can be filled out prior to your arrangement conference and will be emailed to the your funeral director. This will help us better prepare to serve you during your meeting.

Decedent's Personal Information


Father's Name and Mother's Name (Maiden) are required for the death certificate. In the event one or both of these names are unknown, it will appear as unknown on the death certficate.

If the deceased is a veteran, we will need a copy of the military discharge paper (DD214) in order to provide a flag or request any military honors.

Service Preferences

(Please do not plan or publish service date and times until confirmed with funeral home)

Active Pallbearers will be the individuals taking an active role in carrying the casket. They need to be physically able to lift and carry a minimum of 50 lbs.

Honorary Pallbearers are groups or individuals you would like to show honor to by listing them. It is acceptable to list groups or individuals here even if they are unable to attend.

In the event someone would like to donate funds to a charity instead of sending flowers to the services. List the charity below.
